The Initial Meeting

The initial meeting will consist of a consultation of 60-minutes. At the end of this meeting, Dr. Lagos will make recommendations as to what would be the most effective treatment plan. At this time, she and the client will both make the decision about the most effective way to proceed. Prior to your first visit, please download the enclosed forms and bring the completed forms to your appointment with Dr. Lagos.

Insurance Policy

Dr. Lagos requests full payment for services at the end of each session. As a courtesy to clients, she will submit claims to client's insurance or provide a bill with all required information that can be submitted to the insurance for out of network reimbursement. 

Cancellation Policy

In order to insure the ability to provide timely appointments, Dr. Lagos has a 24-hour cancellation policy. Any cancellations that are provided 24-hours prior to the appointment time will not be charged. In the event that the client does not provide notice or cancels with less than 24-hours notice, the full fee will be charged.

Dr. Lagos' office located between 59th Street and Madison Avenue on the Upper East Side of Manhattan.



There is a parking garage located on 59th Street between 5th and Madison Avenue for clients who are driving.

The closest subway train stop is on 59th and Lexington Avenue or 60th and Fifth Avenue.
